Middle school to high school can be a big change for any student. Students are adjusting to a new school, new expectations, more challenging classes, new routines, and increased responsibility. For students with disabilities, these changes can create additional hurdles and make it harder to stay connected and successful in school. Students with disabilities may experience higher unemployment rates, lower attendance rates at college or other postsecondary training, and a 50% higher likelihood of dropping out of school. 

IEP team planning provides the support a student with disabilities needs to enter middle school or high school with a clear plan for learning, earning, and living. Beginning at age 14 the process of transition services makes specific plans for educational support related to community involvement, independent living, lifelong learning, and employment. When schools and families work together to prepare students, they can reduce anxiety, identify needed supports, and help students feel more confident about the changes ahead.
