Post-Secondary Goals versus Transition Goals

Post-Secondary goals should measure what the child/adult is going to accomplish after
high school. This can include employment, college, on-the-job training,independent
living skills, etc.

Transition goals measure what the child/adult is going to accomplish right before they
graduate high school. This might include job shadowing, completing job interviews,
applying to college, etc. Transition goals help the student prepare for post-secondary
goals.


Examples of Post-Secondary Goals:


Education/Training

  • After graduation, Sophia will attend a community college and major in Psychology.
  • After high school, Adam will begin on-the-job training at a [specific company name] construction company with the support of Vocational Rehabilitation.
  • After exiting 18-21 year transition services and the support of Vocational Rehabilitation, Dominique will receive training in his community based on his strengths, interests, and skills to explore job opportunities.

Career/Employment

  • After exiting from 18-21 year transition services, Micah will begin part time employment at the local hospital working as a front desk clerk in the fitness center.
  • While attending a two year college, Jose will work part-time at a pet store while taking classes to become a vet tech.

Independent Living Skills

  • After graduation, Tom will live at home and participate in his daily routines as independently as possible (getting dressed, making meals, and bathing).
  • After high school, Bianca will live independently in an apartment and will access services in the community by using the city bus.
  • After exiting 18-21 year transition services, Jacob will live in a group home with 24 hour support to gain independent skills such as bathing, preparing meals, cleaning, and transportation to community activities.

Examples of Transition Goals


  • Given strategies and instruction, Josiah will participate in 3 job shadows of his interest and write a reflection paper after each job shadow, by 10/05/22.
  • Given strategies and instruction, Maddie will visit 3 community colleges of her interest and know how to apply to college.
  • Lydia will be able to acquire the skills needed to retain a job in a restaurant (being on time, hygiene, customer service).
