What is transition?
Transition is the time when you start getting ready for life after high school. Usually, this happens between the ages of 14 and 21. During these years, you, your family, and your school team work together to make a plan for your future.
Transition planning helps you think about things like:
- Where will I spend my time after school?
- What activities, work, or programs do I want to be involved in?
- What you want to do after high school (college, job training, or other programs)?
- The kind of job or career you might want ?
- Skills you need to live more independently, such as managing money, transportation, or daily living tasks?
- What supports will help me be successful?
The goal of transition planning is to help you build the skills, experiences, and supports you need to be successful as an adult and reach your personal goals.
Purpose
Transition planning helps you figure out what you want to do after high school and what steps you need to take to reach your goals. It focuses on building the skills, experiences, and supports you need to be successful in adult life. Whether your goal is to go to college, get a job, learn a trade, live more independently, or all of these, transition planning helps make sure you are ready for the next stage of your life.
The purpose is to:
- Help you identify your strengths, interests, and goals
- Prepare you for education, training, or employment after high school
- Teach skills needed for adult life and independence
- Connect you with services and supports that can help you succeed
- Give you a voice in planning your future
In short, transition planning is about helping you move from school to adult life with confidence and a clear plan for your future.
